Rates
If you have insurance and wish to pay out of pocket and file your own claim or bypass insurance altogether, you will be charged the provider reimbursement rate according to your insurance plan. If you do not have insurance, fee's begin at $125.00 an hour and can be reduced according to your ability to pay.

Insurance
I accept most major insurance and EAP plans. The only cost for you will be your co-pay, if at all, depending upon your policy. If you have approved sessions from an EAP, services are free of charge.
Many insurance carriers require an authorization number for out-patient mental health, while a few others do not.  Please be sure to contact your insurance to verify your benefits that include deductibles, co-pays, number of sessions per year and the authorization requirement.  If you  need an authorization they will provide you with the number. Please bring that number with you to your appointment. If you have a referral from your employers EAP, you always need an authorization. If an authorization is required and you do not have one, you can not be seen.

Reduced Fee
I offer a sliding scale fee, for special financial circumstances. If necessary this will be taken into consideration at the time I schedule your appointment.

Payment
Cash or check only. Credit cards are not accepted at this time.

Cancellation Policy
If you do not cancel within 24 hours or show for your appointment, you may be charged  the entire cost of your session.
